Individual Supports (IS)

Hearten offers personalized in-home support through the DDD Community-Based Supports (CBS) and Individual Supports (IS) programs. Each participant is matched with a Direct Support Professional (DSP) who provides tailored one-on-one assistance. This individualized approach allows for engaging, person-centered activities that cater to the unique needs and interests of individual, both at home and within their local community.

Behaviors and Medical Accuities:
Hearten specializes in behavioral and medical acuities. We have expertise, resources, and trained professionals to care for individuals with complex behavioral challenges and significant medical needs. Our staff includes:

Behaviors Accuities:
Behavioral acuities refer to significant or complex behavioral issues that may require specialized interventions and management. These behaviors may include:

Elopement: Running away from home, school, or care settings.

Medical Accuities:
Medical acuities refer to complex or chronic health conditions that require consistent monitoring, medical intervention, or specialized care. Examples include:

What is Individual Supports (IS)?
Individual Supports (IS) in New Jersey is a service provided by the Division of Developmental Disabilities (DDD) that focuses on assisting individuals with daily living tasks in their own homes or residential settings. This service aims to help people manage their day-to-day lives while improving their independence.

How to Access Individual Supports in NJ:
To access IS / CBS services, the individual must:​

Be enrolled in one of the DDD Medicaid waiver programs:

Supports Program (SP) – for individuals living with family or independently.

Community Care Program (CCP) – for individuals requiring more intensive support, including residential services.

Have an Individualized Service Plan (ISP) that outlines the goals and the types of IS services needed.
